The role involves post-discharge planning and use of EPIC for electronic medical records. Responsibilities: Manage a caseload of 15-20 patients. Provide resources for post-discharge and community support. Requirements: 2 years of recent acute inpatient case management experience required. 4 years of overall nursing experience required. ADN or BSN (or above) accepted. EPIC experience required.
