Job Description:
Company: Providence at Home with Compassus Position Summary The
Registered Nurse plans, organizes and directs Hospice care and is
experienced in nursing, with an emphasis on community health
education / experience. The Hospice nurse builds from the resources
of the Interdisciplinary Group (IDG) to plan and direct services to
meet the needs of individuals and families within their home
environments. May provide patient care to premature infants,
infants, pediatrics, adolescents, young, middle and older adults.
Position Specific Responsibilities • Completes comprehensive
assessments of patients and families to determine hospice needs,
including physical assessments and histories of current and
previous illnesses. • Develops, initiates, and revises
individualized Plans of Care in collaboration with the IDG, using
health assessment data to determine nursing diagnoses and establish
goals. • Provides professional nursing care by utilizing the
nursing process, implementing palliative interventions,
administering prescribed medications and treatments, and counseling
patients and families on health care needs. • Provides health care
instructions to patients and families as appropriate per assessment
and plan. • Coordinates with the Patient Care Manager and IDG to
ensure continuity of care, safe environments, and support for the
emotional needs of patients and caregivers. • Completes, maintains,
and submits accurate and timely clinical documentation, including
patient condition, care provided, and pain/symptom management
outcomes. • Communicates patient needs, condition changes, and
physician orders to the IDG and supervisors; attends IDG meetings
to review and update Plans of Care. • Fulfills accepted case
assignments, participates in on?call duties, and maintains
productivity standards per organizational guidelines. • Supervises
and delegates responsibilities to ancillary personnel as
appropriate. • Supports the orientation, education, and development
of team members, and provides input on CHHA performance and
assignments. • Assumes responsibility for professional growth
through continuing education, in?service programs, and other
activities to enhance job?related skills and abilities. • Actively
participates in performance improvement initiatives and adheres to
the Corporate Integrity Plan. • Ensures financial prudence in care
delivery by coordinating medications, supplies, equipment, and
other necessary services defined in the Plan of Care. Education
and/or Experience Education requirements noted separately with each
state. Alaska (AK) Required: Graduate of an accredited school of
Nursing. Preferred: Experience in Oncology, Hospice, or Long?Term
Care. California (CA) Required: Education sufficient to meet
certification, licensure, or registration requirements. Preferred:
1 year of recent experience in hospice, palliative care, home
health, acute care, and/or skilled nursing as a registered nurse.
Oregon (OR) Required: Graduate of an accredited school of Nursing
with at least 1 year of full?time experience as a registered nurse.
Preferred: Recent clinical experience (within the last 3 years) in
a Medicare?certified Homecare or Hospice agency/facility performing
intermittent visits and case management. Washington (WA) Required:
Associate’s Degree or Bachelor’s Degree in Nursing, or equivalent
education/experience. Preferred: 2 years of nursing experience,
including at least 1 year in hospice, home health, skilled nursing,
medical/surgical, or oncology. Skills Mathematical Skills: Ability
to add, subtract, multiply, and divide in all units of measure,
using whole numbers, common fractions, and decimals. Ability to
compute rate, ratio, and percentage. Language Skills: Ability to
read, analyze, and interpret general business periodicals,
professional journals, technical procedures, or governmental
regulations. Ability to write reports, business correspondence, and
procedure manuals. Ability to effectively present information and
respond to questions from leaders, team members, investors, and
external parties. Strong written and verbal communications. Ability
to understand, read, write, and speak English. Articulates and
embraces integrated healthcare at home philosophy. Other Skills and
Abilities: Excellent observation, verbal and written communication,
problem solving and basic math skills. Visual acuity and hearing to
perform required nursing skills. Preferred working knowledge with
computer applications, i.e. Microsoft Word/e-mail. Certifications,
Licenses, and Registrations Required within 30 days of hire:
National Provider BLS – American Heart Association (Vendor Managed)
Active state RN license (Vendor Managed). State Specific
Requirements Other Required upon hire: Active Registered Nurse
License (Vendor Managed) Physical Demands and Work Environment: The
demands of this role necessitate a team member to effectively
perform essential functions. Adaptations can be made to accommodate
team members with disabilities. Regular standing, walking, and
manual dexterity are fundamental, along with the ability to lift
and move objects up to 50 pounds. Visual acuity requirements
include close and distance vision, color and peripheral vision,
depth perception, and the ability to adjust focus. In a healthcare
setting, exposure to bodily fluids, infectious diseases, and
conditions typical to the field is expected. Routine use of
standard medical equipment and tools associated with clinical care
